This page provides links for downloading the most requested forms (applications, affidavits, etc.) provided by our office. If you need a form which is not listed here, then either e-mail us or call us at 713-368-2000, and we'll help you obtain that form.

You may view the following forms in Acrobat format by clicking on the appropriate link. You will need Adobe Acrobat Reader in order to download and view the forms. If you do not have Adobe Acrobat Reader, you may download the latest version from Adobe Software.

To save the file to your PC: (1) right click on the link, (2) select "Save Target As..." (Microsoft Internet Explorer) or "Save Link As..." (Netscape Navigator), then (3) choose a directory and filename for your PC.  If you wish to download another form (after having viewed a previous form) then click the "Back" button on your browser to get back to the "Forms" page.

Please note:  Our site does not have Texas Motor Vehicle and Title Registration forms, nor Texas Parks and Wildlife boat-related forms.
